 Exceptional Orthodontic Results  齊杏齒顎矯正專科-非手術改正歪斜

五十年來，自從 Dr. Hugo Obwegeser 在 Waler Reed Army Hospital 為美國士兵執行正顎手術，開啟了美國齒顎矯正專科醫師與外科醫師的成功合作篇章，矯正合併正顎手術已經歷經許多臨床診斷、技術、術後追蹤的成功經驗。

然而，仍有些患者，於手術前跟改了矯正合併手術的計畫，選擇臨陣脫逃，而讓身為矯正醫師的我既無法拒絕治療，更必須獨立肩負改善外觀的重責大任。

    矯正治療中，我們拔除原本可能不需要拔除的下顎第一小臼齒以用來改善下唇外突。讓臉型更對稱與平衡，並使用迷你骨釘來做錨定作用。矯正治療後，可以發現外觀的重要改變，鼻唇角的變大使得鼻型極為自然與立體，下巴因為下顎門牙的後移而使下唇得到可以放鬆使得形態更為美觀。

我們不希望矯正的目的是為了齒列整齊而已，同樣是矯正治療，但是經過了二年、五年、甚至十年，這樣的排列給了患者什麼樣的表情與意義？我們希望患者是自信的、是感謝的。我們追求細節，就是對追求-美麗臉部自然的尊崇。這樣才有可能雋永。』

矯正不滿意常常是矯正醫師非常頭痛的問題，然而相信承受這些壓力與身心煎熬的更重的可能是病患本身。要如何避免這類的情形發生？其實一開始與醫師之間的諮詢溝通時就要有警覺，是否矯正醫師本身是『中華民國齒顎矯正學會』的專科醫師？是否充分了解該醫師的治療案例與結果？並有完善的溝通？矯正醫師是否有完整解釋咬合與顏面可能發生的情形，好的與不好的，而非只強調矯正商品的優點等等。矯正發生內傾仍然出現在許多拔牙的案例之中，牙齒必須回復到正常角度，再將上顎牙齒後移或移動至理想的位置，笑容才能重建。

    本文下方的案例是經過矯正之後，發現自己的前牙變得『太直』，並且在不笑的時候有『苦瓜臉』的感覺。而大笑時，卻又『露牙齦』！

     事實上，就診斷來說，是由於矯正後上顎前牙的位置太過往下，前牙的角度太過朝內旋轉（矯正醫師稱為 loss torque control，翻譯喪失扭矩）。我們矯正的解決方案是將前牙反向轉回以外，更需要將整個齒列後移，才能彌補掉前牙外翻一些些後，所回復的暴牙。（暴牙事實上沒被解決，因為上一次的矯正是把牙冠後移，而非牙根！）
