    這位患者遠從外地來初步咨詢時，對我們診所所能達到的矯正效果其實已經了解許多。但他認為自己的嚴重程度很高，依然不放棄『非手術』的純矯正。

    由矯正用的側顱X光片即可看出下排前方齒列完全在上排牙齒的外面，咬合完全無法閉合！嘴巴也無法和起！而患者也為這樣的咬合與顏面外觀困擾很久！

    矯正醫師對於歪斜與開咬，一直視為非常困難的案例之ㄧ。更何況希望葉醫師利用純矯正來改正。

    When we look at the lower dentition, there are many space between different teeth. So, if we close all of them and try to move the whole dentition back, the extraction is avoid!!!

    The correct diagnosis and treatment planning is more important than everything! Because after the most decent treatment planning and discussion, you will understand where is the limit of the orthodontic treatment. We don't want any subjective thinning instead of not looking at the data. It's a life-long issue!

『非拔牙解決上顎嚴重擁擠和下顎外突』

    近日有可愛的年輕患者尋求戽斗（反咬）的矯正治療，問及是否該合併手術的矯正方式。事實上，矯正醫師並無法改正骨頭的長度，僅能改正齒列及其周圍支持組織，然而對於抗拒手術的患者，我們於是尋求非手術又必須兼顧顏面美觀的解決方案。對於希望徹底改變骨頭長短，而有所謂變臉效果的患者，當然合併正顎手術的矯正治療，為最迅速有效的解決方案。長期與外科醫師合作的我，也非常熟稔於該治療，並可提供理想的外科醫師與您溝通顏面。

上顎擁擠幾乎將側門牙蓋住的牙弓，看似必須經過拔牙，才能得到最好的解決效果。卻因為她原本太過凹陷的鼻側，讓我決定將上顎前牙帶出，以讓上唇飽滿。並將下顎前牙適度後移，以創造出更為勻稱的外觀。

"Non-surgical approach to resolve severe crowding of upper jaw and protrusive mandible"

In the following case, the severe dentition almost makes the lateral incisors  "invisible". Almost most orthodontists will choose extraction to have the easier result. However, I did not extract any tooth in order to make the front teeth forward to have the fullness of upper lip. To push the lower anterior teeth backward, to have the more balance and symmetrical face. 

『拔牙解決外突上下唇關係』

相較於太過於前方的反咬戽斗案例，在外觀首先考慮的治療美學前提，同樣可以得到患者需求的矯正結果。沒有了侵略性的手術，對沒有接受讓人認不出的整形手術的患者中，猶如畫作般，協調與簡單的優雅曲線，一樣有令人驚歎的矯正效率與結果。

"Extraction to resolve the protrusive upper and lower lip"

Compared to the previous case, the "face-oriented" treatment philosophy help me decide to do extraction to have the better lip relationship.  In the world of pure orthodontic treatment without invasive surgery, we also can have astonished, like the painting, balance and simple elegant cures to describe best orthodontic results.

    對於下顎骨突出、前牙錯咬的第三級咬合不正，我會仔細分析模擬圖後告知外觀改善的幅度，在有限的情況底下，許多案例也許可以以較為輕鬆的不靠手術解決。在此分享快速矯正完成的案例，由治療計畫擬定，收集資料，至完成給予維持器，短短12月完成。

    Many protrusive lower jaw with anterior crossbite patients might deny surgical orthodontic treatment.  However, I'd like to clarify that "pure orthodontic treatment" alone can't change the lower jaw itself, but  affect the contour of the alveolar bone that cover the teeth. The following case was done by pure orthodontic treatment without extraction any tooth. The whole treatment process includes the data collection and delivery of the retainer is with 12 months.

    由於牙齒的移動事實上只能影響周圍的齒槽骨，而非下顎骨本體，所以下顎骨的最凸點無法改變，而該處骨頭的突出常常是造成下顎前牙不適合移動到太後方的位置，容易造成下巴比例變得過於明顯。讓我不得不停留在『最和諧、最對稱』的位置，而非將牙齒後移至最遠處。該病例診斷分析後，並無拔除任何牙齒，採用預期性的全齒列移動，包含上顎前牙，因為全齒列移動，我才能將擁擠的上顎齒列排整齊，而沒拔牙齒。許多醫師會認為改善下顎牙齒錯咬已經非常棒了。但我不這麼認為，唯有根據臉型而擬定的上下牙齒全齒列移動，也才能改善對病患本身最重要的鼻唇角大小、微笑、角度和唇下顎角，做到矯正零缺點。

    Since the orthodontic treatment only can modify the dentoaveolar bone, the most prominent point of the mandible can't be changed and it's always the main role that affects me to decide where the lower anterior teeth are when considering the harmony and symmetry. The upper anterior teeth also need to be distalized totally, in oder to control the angulations. Many doctors will think push back the lower teeth is good enough for the patient's facial profile. However, I don't think that way. Moving the both arch back according to the face is very important for the patient's esthetic and smile. Then, we can call it the best result.
